Evidence of pneumothorax on radiograph of the chest without accompanying major respiratory signs is rare. Various artefacts including skin fold, shirt fold, bed sheet fold and medial border of scapula are known to mimic pneumothorax. Though detection of lung markings in the periphery rules out pneumothorax, but in conditions such as emphysema and bullous disease it is difficult to these markings. It is important to differentiate between a pneumothorax and a pseudopneumothorax because unnecessary intervention could precipitate serious complications.
