A 53-year-old male patient, presenting merely with fever for a week, was admitted to the hospital. Contrast-enhanced CT showed a round inhomogeneous enhancement lesion on the early arterial phase and washout in the portal phase (figure 1A,B). Hepatitis virus tests and tumour markers in serum were all negative. Considering the atypical imaging features of intrahepatic space-occupying lesions, the possibility of liver tumours or liver abscesses is not excluded. The patient underwent combined multisegmental hepatectomy. Postoperative pathology revealed hepatic perivascular epithelioid cell neoplasm (PEComa) (figure 2A).
