Bilateral serous macular detachments in Waldenströms macroglobulinaemia

A 71-year-old man presented to the emergency department with reduced vision in the right eye of 4 weeks duration. He complained of hazy and distorted vision, shortness of breath, and feeling light-headed. At presentation his visual acuity was count fingers in the right eye and 6/9 in the left eye. Fundus examination revealed bilateral, highly elevated, serous macular detachments and tortuous vessels with diffuse flame-shaped retinal haemorrhages, worse in the right eye (fig 1). The degree of macular elevation was confirmed by an optical coherence tomography (OCT) scan (fig 2A).
