Multiple xanthomas in a young male with familial hypercholesterolaemia

A 15-year-old boy with a history of familial hypercholesterolaemia was admitted for a check up. The diagnosis of hypercholesterolaemia was made at the age of 2 years. He had a strong family history of premature coronary artery disease, with one of his first degree relatives dying at the age of 17 years.
On examination, his weight was 42 kg, height 152 cm, body mass index (BMI) 18 kg/m2, and blood pressure 110/60 mm Hg. He had evidence of bilateral tuberous red yellow xanthomas over both the elbow (fig 1A) and ankle joints (fig 1B) without arcus cornea. Tuberous tendon xanthomas of tumorous proportion were noted over the Achilles tendon (fig 1C), flechissor of the toes (fig 1B), and flechissor of the hands (fig 1D).
